Before you start, please read this
Who is asking. This form is run by Indigo Girl Foundation, a
registered non-profit organisation in South Africa, backed by a pro bono legal team.
This will be contributing to legal action that will be taken against the institutions.
Our Information Officer is Indigo Haddington, and you can reach her at
indigo@indigogirlfoundation.com .
What we are asking for. Only what is on this page:
How to reach you
Your province
The capacity you are coming forward in
If you choose to, what happened to you, when, and whether it went to the police,
the prosecutors or a court
Why we ask what happened. We are collecting accounts as evidence for
the legal action against the institutions that failed women. What you tell us about
your experience is sensitive information. We use it only for that legal action. It is
read only by our own team and the pro bono lawyers working on the case, and we never
publish it or give it to the media. We hold it only if you tick the separate box that
says you agree.
Please do not send dockets, case numbers, medical records or
photographs. We only ask whether you have a case number, not what it is.
It is voluntary. You do not have to fill this in and you do not have
to answer every question. If you leave out a way to reach you, we cannot reply to you.
That is the only consequence.
Your rights, and how to complain
You may ask us to confirm what personal information we hold about you, free of charge.
You may ask us to correct or delete anything inaccurate, irrelevant, excessive or out of date.
You may object to our processing, and you may withdraw consent at any time.
Only our own team sees this, and only those who need to. We do not sell your
details, we do not give them to donors, and we do not give them to the media.
You may complain to the Information Regulator of South Africa, JD House,
27 Stiemens Street, Braamfontein, Johannesburg, 2001, on Form 5.
Our Information Officer is Indigo Haddington. Write to
indigo@indigogirlfoundation.com
to ask what we hold, to correct it, or to have it deleted.
Please read this before you go any further.
We cannot take on individual cases, and we would rather tell you now than let you
wait months on us. The organisations listed under
need help today do assess individual matters and can
guide you. You are still very welcome to add your name below for updates. Just
please do not wait on us for help with your own matter.
One of the two is enough. Whichever is safer for you.
Anything we should know about reaching you safely?
Optional. Good times to call, for instance. If you want to tell us what happened, there is a separate place for that below.
Your experience Optional
Answer only what you want to. You can leave any of it blank, or all of it.
Take your time, and stop whenever you need to.
Time limits apply to claims against the State.
Some are as short as six months, and telling us does not stop them. We cannot tell
you what your own dates are. Please speak to Legal Aid South Africa or one of the
organisations under
need help today about your own
matter as soon as you can.
What happened to you?

Is there anything else you want to share with us?
I agree that Indigo Girl Foundation may hold what I have written about my
experience, which is sensitive information, and use it for the legal action against
the institutions. Needed if you answered anything above
Submitting this does not make anyone your lawyer, does not guarantee
representation, and does not begin legal proceedings.
